With plastic surgery so mainstream the question today isn’t “did she or didn’t she?” but “what did she have done?” Botox, filler, liposuction, facelift, breast implants, are not just limited to celebrities as they once were. There isn’t a body or facial concern that can’t be addressed through some sort of procedure. With more and more plastic surgeons striving to educate people on cost, safety, recovery process, and long-term care; Dr. Stanley Poulos, San Francisco Bay area Board Certified Plastic Surgeon answers some of the most common and pressing cosmetic surgery questions today.

Q: Will liposuctioned fat return?

A: Dr. Poulos explains that, “If the patient gains a small amount of weight after their procedure, say 5 pounds, fat cells throughout the body will get a little bigger. While this slight weight gain can diminish results, the overall improved body shape provided by a liposuction procedure will still be visible as treated body areas have fewer fat cells (and thus experience a lower enlargement rate) compared to surrounding areas.”

Finally, in cases of considerable weight gain (i.e. 10% of their body weight); new fat cells can develop in all areas of the body, including treated areas. However, since new fat cell creation typically occurs in an even manner throughout the body, fat often accumulates less in treated areas compared to untreated areas. Many liposuction patients who gain considerable post-procedure weight tend to still look better than if they did not undergo liposuction.

Q: Will you lose sensation in your breasts or nipples if you get implants?

 A: Dr. Poulos says that, “There’s a chance, although it’s very unusual to lose feeling in your entire breast. About 5 percent of patients experience numbness in their nipples. This may occur if their nerves are cut or stretched during surgery.”

 Q: How close do I need to be to my “ideal weight” in order to have the optimal result from liposuction?

A: Dr. Poulos stresses that, “The key thing to know about liposuction is that it is not intended to be a weight loss tool or method. It is simply to address the parts of the body that are not responding to diet and exercise. That said, the industry standard for liposuction is a candidate who is within at least 30% of their ideal weight and are of average body weight. Other things to consider for an optimal result are skin elasticity and muscle tone.”

Q: How long does a Botox treatment last?

A: According to Dr. Poulos, “One treatment may last 3-4 months, but may not result in a significant improvement of the wrinkles for the long term. One or two years of consistent Botox in the same area will result in long term reversal of some wrinkles and optimize the improvements you may be looking for.”

Dr. Poulos adds, “It is recommended to repeat Botox injections every 4 months for maintenance. As you continue to use Botox, deeper wrinkles will continue to soften as dermal remodeling takes place.”

 Q: If a partner squeezes your implants too hard during sex, can they burst or alter shape?

A: Dr. Poulos reassures that, “It’s impossible. He’d have to use superhuman force to rupture them. And they won’t change shape.”

Q: Are there any non-invasive procedures that can sculpt my body, so I don’t have to get liposuction?

 A: Yes! The most well-known non-invasive option is Coolsculpting. With this procedure fat cells are frozen in a way that does not damage other tissues and spot areas of excess fat can be sculpted. Q: How long do the results of a facelift last?

Dr. Poulos says that, “One thing is for certain and that is that the aging process goes on regardless of the technique. Eventually if one wishes to maintain the improvement, touch up procedures will be necessary. An average time might be 7-10 years. Those that address the deeper muscle layers do last longer, but there is more difference (in longevity) between individuals than techniques, and the same is true of time for recovery.”

 Q: I lost over 50 pounds and now have excessive loose skin on my stomach, arms and inner thighs. What’s my best option and what’s involved?

A body lift is the most common option when addressing excessive skin after significant weight loss. It’s important to research this procedure and go about it in a way that makes sense given your overall health. Body lift offers a permanent result provided weight is kept the same. The thing to consider with a body lift is a high Body Mass Index and potential for abnormal scarring and fluid buildup. Be sure the surgeon shares all risks and benefits.

 Q: Should I get saline or silicone breast implants?

There are two basic types of breast implants: saline and silicone gel.

 Dr. Poulos says that, Most women feel that silicone implants feel more like natural breasts which is due to the viscosity of the filler material. Previous concerns about the association of auto immune disease with silicone have been largely disproven and the vast majority of plastic surgeons feel that silicone is a perfectly safe alternative.”

New treatments and procedures claiming to blast fat and smooth skin enter the market regularly, offering the possibility of a contoured, toned body. Many people who exercise regularly, and watch what they eat find stubborn weight collecting anywhere from the back, arms, abdomen, hips, thighs and derrière. Frustrated, they seek solutions, but which ones deliver and how safe are they? To give us answers, we turned to Dr. Stanley Poulos, Board Certified Plastic Surgeon in the San Francisco Bay area and co-founder of Plastic Surgery Specialists who has over 3 decades expertise in body aesthetics.

What is driving women’s decisions to try these treatments and how have attitudes changed from a decade ago?

Ten years ago, women would come to my office with photos of celebrities who were working with highly effective fitness trainers. Today both women and men come to my office, show me a random “hot body” in their Instagram feed, looking fabulous in a swimsuit, and want to look like that person. Social media bombards people with images, and they feel if these people aren’t well-known celebrities and look great, then they can too.

What are the most common frustrations when it comes to achieving their desired look?

Well, most people try their best to work out, watch what they eat and try to take care of themselves. They notice, though, that through pregnancies, hormonal and metabolism shifts that happen with age, gravity is winning. Men notice more fat around the waist and “breasts”, women notice cellulite dimples in their thighs and buttocks, bra rolls, extra fat at the waist despite maintaining their diet and exercise routines. A workout routine at 45 will not yield the same results as 35; it becomes frustrating. Collagen production typically falls off a cliff at a certain point in the early 50s; this means treatments need to treat both fat deposits and skin tightening.

What do you advise them to do? There are so many treatments to choose from. It can be overwhelming.

Absolutely, and it’s very important to approach any procedure with the facts and a realistic expectation of what’s achievable, and required.. Some women think they just come in for a few hours and they are magically transformed. In some cases, results are immediate, and in other cases healing can take up to 2 to 3 months.

Ok so let’s explore these treatments. What’s the best one out there to deal with cellulite? It seems like most women struggle with cellulite.

 Cellulaze

Yes, cellulite is a very common issue women want a solution for, and fast. Cellulaze is a treatment that safely and effectively treats cellulite under local anesthesia with initial results appearing in one to four weeks, with significant results due to thickening collagen by four months. The 2-hour treatment involves an injection of a local anesthetic and wetting solution onto the cellulite, which is then zapped with a laser. This unique side-cutting laser removes the contracted fibrous bands responsible for the cellulite. Finally, the subdermal region is treated for increased collagen production. The result is tight, smooth, supple skin where the cellulite once was. Many women opt to get Cellulaze along with liposuction to address several concerns at once.

Vaser Hi- Def liposculpture

Yes, let’s talk about liposuction. How has that treatment changed?

Well, I’ve seen significant improvements over the years. Now, we have a much more precise method called Vaser Hi-Def LipoSculpture. This method is liposuction, but with techniques and tools that precisely suction out localized fat deposits from various parts of the body. This technique is used effectively, for example, in “sculpting” the washboard abs so desired by many. The fat that is harvested can then be used to fill out other areas of the body. The differentiating factor between Vaser Lipo and traditional Liposuction is the preliminary step taken to break down the fat: ultrasound frequencies. The acronym Vaser stands for Vibration Amplification of Sound Energy at Resonance.

The vibrations emitted cause expansion and contraction of air bubbles located in tumescent fluid which is ejected from the Vaser Lipo instrument. The procedure is tissue-selective, targeting fat while protecting other tissues from damage. This means eight times less blood loss, easier fat removal, stimulated skin elasticity, less bruising and a tiny incision. Through this process, the fat that is collected is separated to use for fat injections in other areas. The surgeon has the ability to truly sculpt the body.

Balloon Weight Loss Method

What about people who have 20-50 pounds to lose, but not enough weight for gastric bypass surgery?

 These patients can be considered for the Balloon Weight Loss Method. An ideal candidate for this procedure has a body mass index of 30 to 40 and did not had previous weight loss surgery. Patients diagnosed with bulimia, binge eating, compulsive overeating, high liquid calorie intake habits or similar eating related psychological disorders are not good candidates.

This non-surgical outpatient procedure begins with a diagnostic endoscopy, to ensure that there are no contraindications and that it is safe to perform. Once the patient is mildly sedated and comfortable, the procedure can begin. The deflated gastric balloon is inserted through the esophagus and into the stomach. A syringe is then used to fill the balloon with a sterile saline solution. Once the weight loss balloon has been filled with saline, it expands to approximately the size of a grapefruit and remains there for 6 months. The entire procedure takes about 20 minutes. Patients can usually return home after the placement or removal procedures within 30 minutes. Over the last 20 years this procedure has helped over 277,000 people. The gastric balloon encourages portion control while patients make healthy changes to diet and lifestyle. After six months the balloon is then removed.

Want a nose job, a tummy tuck or some other cosmetic surgery or procedure? So do 17.5 million other Americans, or at least they did in 2017.

The American Society of Plastic Surgeons recently released its annual survey that reveals how many people are undergoing plastic surgery and what kinds of procedures they are seeking. The report showed a 2 percent increase in the number of surgical and minimally invasive cosmetic procedures performed in 2017 as compared to 2016.

“For some people plastic surgery probably still has a stigma attached to it, but millions more realize that improving yourself is okay and should be accepted,” says Dr. Deepak Raj Dugar (www.scarlessnose.com), a world-renowned Beverly Hills plastic surgeon who specializes in the Scarless Nose™, or closed rhinoplasty, procedure that leaves no sign of an incision.

Dugar says a few things to consider for anyone who is considering plastic surgery include:

  • Do you really need to change? Sometimes people are determined to change something about their bodies that really doesn’t need changing. “Not everyone needs surgery,” Dugar says. “I actually on occasion tell patients that their nose looks fine, they should leave it the way it is.”
  • Be sure you know what you’re getting. Patients can be disappointed if they and the plastic surgeon aren’t clear with each other on what to expect. “The patient needs to understand the limitations and goals of the surgery before undergoing it,” Dugar says. “For example, you might be expecting a dramatically different look, but the changes are actually much more understated. The only thing I get accused of is doing too little to a nose, and I’m totally okay with that.”
  • Safety is critical. “It’s important that your plastic surgeon picks safety over results,” Dugar says. “Safety has to and always should come first.”

The top five cosmetic surgical procedures in 2017 were, in order, breast augmentation, liposuction, nose reshaping, eyelid surgery and tummy tucks.

Meanwhile, the most popular minimally invasive procedures were Botox, soft-tissue fillers, chemical peel, laser-hair removal and microdermabrasion.

Dugar says one of the key features of successful plastic surgery is subtlety.

“The secret of Hollywood celebrities is doing Botox with plastic surgeons who understand the artistry behind the science,  where less is more,” he says.

Many Americans don’t want to go under the knife to achieve the changes they seek. The ASPS reported 15.7 million minimally invasive procedures were performed in 2017. Surgeries accounted for the remaining 1.8 million procedures.

Dugar says plastic surgeons often concentrate on specific procedures, as he does with closed rhinoplasty, so anyone considering cosmetic surgery should seek a surgeon whose experience lies in the procedure they desire.

“You don’t want the person who specializes in noses doing your breasts,” he says, “and you don’t want the one who specializes in breast augmentation doing your nose.”
