Plastic Surgery Everything You Wanted To Know But Didn’t Know How To Ask

With plastic surgery so mainstream the question today isn’t “did she or didn’t she?” but “what did she have done?” Botox, filler, liposuction, facelift, breast implants, are not just limited to celebrities as they once were. There isn’t a body or facial concern that can’t be addressed through some sort of procedure. With more and more plastic surgeons striving to educate people on cost, safety, recovery process, and long-term care; Dr. Stanley Poulos, San Francisco Bay area Board Certified Plastic Surgeon answers some of the most common and pressing cosmetic surgery questions today.

Q: Will liposuctioned fat return?

A: Dr. Poulos explains that, “If the patient gains a small amount of weight after their procedure, say 5 pounds, fat cells throughout the body will get a little bigger. While this slight weight gain can diminish results, the overall improved body shape provided by a liposuction procedure will still be visible as treated body areas have fewer fat cells (and thus experience a lower enlargement rate) compared to surrounding areas.”

Finally, in cases of considerable weight gain (i.e. 10% of their body weight); new fat cells can develop in all areas of the body, including treated areas. However, since new fat cell creation typically occurs in an even manner throughout the body, fat often accumulates less in treated areas compared to untreated areas. Many liposuction patients who gain considerable post-procedure weight tend to still look better than if they did not undergo liposuction.

Q: Will you lose sensation in your breasts or nipples if you get implants?

 A: Dr. Poulos says that, “There’s a chance, although it’s very unusual to lose feeling in your entire breast. About 5 percent of patients experience numbness in their nipples. This may occur if their nerves are cut or stretched during surgery.”

 Q: How close do I need to be to my “ideal weight” in order to have the optimal result from liposuction?

A: Dr. Poulos stresses that, “The key thing to know about liposuction is that it is not intended to be a weight loss tool or method. It is simply to address the parts of the body that are not responding to diet and exercise. That said, the industry standard for liposuction is a candidate who is within at least 30% of their ideal weight and are of average body weight. Other things to consider for an optimal result are skin elasticity and muscle tone.”

Q: How long does a Botox treatment last?

A: According to Dr. Poulos, “One treatment may last 3-4 months, but may not result in a significant improvement of the wrinkles for the long term. One or two years of consistent Botox in the same area will result in long term reversal of some wrinkles and optimize the improvements you may be looking for.”

Dr. Poulos adds, “It is recommended to repeat Botox injections every 4 months for maintenance. As you continue to use Botox, deeper wrinkles will continue to soften as dermal remodeling takes place.”

 Q: If a partner squeezes your implants too hard during sex, can they burst or alter shape?

A: Dr. Poulos reassures that, “It’s impossible. He’d have to use superhuman force to rupture them. And they won’t change shape.”

Q: Are there any non-invasive procedures that can sculpt my body, so I don’t have to get liposuction?

 A: Yes! The most well-known non-invasive option is Coolsculpting. With this procedure fat cells are frozen in a way that does not damage other tissues and spot areas of excess fat can be sculpted. Still ,as with lipo one must have reasonable skin elasticity to achieve good results.  Another non -invasive option is the V-shape Accent Prime . Q: How long do the results of a facelift last?

Dr. Poulos says that, “One thing is for certain and that is that the aging process goes on regardless of the technique. Eventually if one wishes to maintain the improvement, touch up procedures will be necessary. An average time might be 7-10 years. Those that address the deeper muscle layers do last longer, but there is more difference (in longevity) between individuals than techniques, and the same is true of time for recovery.”

 Q: I lost over 50 pounds and now have excessive loose skin on my stomach, arms and inner thighs. What’s my best option and what’s involved?

A body lift is the most common option when addressing excessive skin after significant weight loss. It’s important to research this procedure and go about it in a way that makes sense given your overall health. Body lift offers a permanent result provided weight is kept the same. The thing to consider with a body lift is a high Body Mass Index and potential for abnormal scarring and fluid buildup. Be sure the surgeon shares all risks and benefits.

 Q: Should I get saline or silicone breast implants?

There are two basic types of breast implants: saline and silicone gel.

 Dr. Poulos says that, Most women feel that silicone implants feel more like natural breasts which is due to the viscosity of the filler material. Previous concerns about the association of auto immune disease with silicone have been largely disproven and the vast majority of plastic surgeons feel that silicone is a perfectly safe alternative.”

Halloween is just about a week away, kids will be eating all the candy they can find and sugar levels will be through the roof. But how much sugar is too much, and what are the signs your child is sugar overdosing?

How Much Sugar Is Too Much?

 

For children, it depends on their age and their calorie intake, but as a general rule of thumb, 12-25 grams a day is just enough.

 

What Are the Signs of Too Much Sugar?

 

#1: Sugar High

 

A sugar high happens when there are extremely high levels of blood sugar, which results in high energy and shaking of the hands. The candy equivalent is three Butterfingers, which is 30 grams.

 

#2: Sugar Crash

 

A sugar crash usually happens within an hour, and glucose levels in the body will drop, causing energy to decline as well. During a crash, you’ll most likely be fatigued, hungry, and may also have a headache. To get rid of a crash, your child’s best bet is to eat an apple or banana.

 

#3: Your Child is Craving More

 

Sugary foods release dopamine, which is the same chemical released during drug use.  Once your child starts eating more sugary foods, such as those Halloween treats, the craving for more will continue.

 

When you eat a lot of sugar, your blood sugar levels rise, which causes the pancreas to produce insulin, a hormone that prompts cells to absorb blood sugar for energy and storage. When you have too much insulin, your blood sugar crashes, which makes you crave more sugar. So, if you cut back on sugary food, you won’t make a lot of insulin, and then you will stop craving the sugar.

 

Choose lower carb, lower glycemic foods, like berries, veggies, proteins, protein shakes, protein bars and yogurt. Pears are a great alternative, since it has almost 14 grams of sugar and reduces the risk of heart disease.

 

#4: Sleeping is Almost Impossible

 

Serotonin is released, which is our relaxation hormone, and is needed for a child to have a good night’s sleep. If insomnia issues arise, it’s time to reduce sugar intake.

Pilates is meant to stretch the body, and it is the thing that you can actually use with machines that will stretch your joints.  You will start to grow because you have forced your body to do so, and you will start to get taller if you have also made sure that you have used the other tips above.

 

Someone who wants to have a taller body has to do something that is going to help them gain height quicker.  They can start a system, and they can use all the tips above to get the results that they need.

Here’s what is certain: There are no firming or tightening products whose results can duplicate what you derive from in-office procedures such as dermal fillers, Botox, lasers, or cosmetic surgery. Board– Certified Denver plastic surgeon Manish Shah says, “There are anti- aging products or ingredients that do perform significantly better than others and can make a profound difference in the skin’s appearance while others are simply “false hope in a jar.”

 

Why don’t most products work the way they claim? Almost without exception, when you buy a product claiming to tighten skin, its effects, if any, are due to ingredients such as film-forming agents. Just like the name states, film-forming agents form a film on the skin, and that can make the skin “feel” tighter. The effect is temporary and you won’t see noticeable lifting of sagging skin, but the sensation is often enough to convince women that the product is working. Dr. Shah stresses that, “Skin “feeling” tighter is not the same as making a real change for the better in the tone or laxity of your skin. Using what really works will get you closer to the results you want.”

 

THE HEROES

 

Dr. Shah says, “Sunscreen SPF 30 or above is the best antiaging cream you can use.  It prevents skin cancer as well as UVA rays that cause loss of collagen and wrinkles in the skin.  Reapply every 2 hours, use a powder sun block and carry in your pocketbook for quick and easy sunscreen touchups.  My favorite powder sunblock is Colorescience, and my favorite base sunscreens have titanium or zinc oxide in them as active ingredients.”

 

Retinoids work great to reduce fine lines and wrinkles and help boost collagen production,” says Dr. Shah.”  He adds, “Over the counter retinols are good, Neutrogena makes a good one, or prescriptions such as Retin- A.”

 

Glycolic acid also reduces fine lines and wrinkles and helps fade pigmentary damage from the sun.  “I often have patients alternate nights with retinoid and glycolic,” says Dr. Shah.

 

Vitamin C is a great antioxidant which reduces free radical damage and is great for anti-aging and helps brighten the skin.  Dr. Shah often recommends it in the morning, underneath sunblock, or at night if patients are too sensitive to tolerate retinoids and glycolic acid.

  

Ceramides are a type of lipid found in the membrane of cells. Dr. Shah states that, “They help hold skin cells together, forming a protective layer that plumps the skin and retains moisture. Ceramide levels decrease as we age which leads to loss of hydration, less skin turnover and dryer, more damaged skin. Replenishing the skin’s ceramide levels will help restore moisture and fortify the skin’s natural barrier, helping skin look and feel younger.” So use a ceramide containing moisturizer for your whole body daily.

 

Hydroquinone works to even out sun spots, blotches, and mottled skin. A little hydroquinone goes a long way. “It’s the most effective ingredient for bleaching skin,” says Dr. Shah.  Hydroquinone fades hyperpigmentation by blocking the enzyme that triggers melanin production in the skin.

 

Green Tea Extract is loaded with nutrients called polyphenols, which have been shown to fight free radicals. It is an amazing antioxidant. Studies have found that ingredients in green tea can reduce sun damage and may protect against skin cancer when applied topically. Dr. Shah offers that, “Using green tea extract under sunscreen can provide an extra dose of protection. Polyphenols in creams and lotions may help slow signs of aging, reduce sagging skin and decrease wrinkles.”

 

THE ZEROES

 

The majority of anti-aging creams are still based on moisturizers such as mineral oil.  Wrinkles look worse when they are dry, so any kind of moisturizer helps, but it’s only temporary and doesn’t address the root cause of the wrinkles such as collagen loss, free radical damage, sun damage and environmental factors.  Don’t be fooled by the anti-aging labels.  Unless there is an actual “active ingredient” such as retinol, the benefit is just moisture but nothing else.

 

B Vitamins Many forms of vitamin B (like B12) can only be absorbed in the small intestine, so no matter how much is loaded into your moisturizer or serum, it’s not going to make a difference. “Vitamins like niacin can have an effect on the skin’s texture and color, but your skin can’t absorb them,” Dr. Shah explains. If you really want to tap into the power of vitamin B to improve your skin’s glow and appearance, stick to eating leafy greens like spinach, asparagus, beans, and peas.

 

Caffeine.  Much like a Starbucks Latte for your brain, caffeine in skin creams can give a boost to your skin, too. Until it wears off.  “Caffeine can temporarily reduce puffiness, especially around your eyes,” explains Dr. Shah.  “But don’t expect permanent results.”

 

Botanical extracts are ingredients extracted from plants (flowers, roots, stems, trees, etc.) for use in skin care for everything from healing blemishes to reducing fine wrinkles. They have been used for centuries and have anecdotal purposes in many cultures. The issue: Botanical extracts need to remain on your skin in order to work. Dr. Shah explains that, “In cleansers, there is simply not enough contact time on your skin for any true anti-aging benefit to take place. Another issue is that most botanical extracts are water soluble, which means that the moment you wet your skin and begin to wash your face, they’re watered down and rendered useless.”

 

Collagen and elastin in anti-wrinkle products. Collagen and elastin in skin-care products can serve as good water-binding agents, but they cannot fuse with your skin’s natural supply of these supportive elements. In most cases, the collagen molecule is too large to penetrate into the skin. But even when it is made small enough to be absorbed it cannot bind with the collagen existing in skin, and there isn’t any research indicating otherwise.
